PC-PTSD-5 PTSD Screening

Screen for symptoms associated with post-traumatic stress during the previous month. The questionnaire is scored entirely inside your browser.

This questionnaire is a screening tool.

It identifies individuals who may benefit from further PTSD assessment. A positive screen does not independently establish a diagnosis.
