Holliday–Segar overestimates fluid needs in some neonates, post-operative patients, SIADH, cardiac failure, severe acute malnutrition (SAM) and meningitis — use restricted regimens (e.g. 60–70% maintenance) per AAP 2018, NICE NG29 and WHO/IAP guidance. Always prefer isotonic crystalloids (0.9% NaCl, RL, plasmalyte) for routine maintenance in children > 28 days; hypotonic fluids carry hyponatraemia risk. Confirm prescription, drug compatibility and infusion device settings independently.
