How to make a prescription pad for your clinic

A prescription pad shows your clinic's identity, tells the pharmacist who issued the prescription, and leaves clear space for the medicines and signature. Work through the steps above: the preview shows the exact PDF you will download, so what you see is what prints.

This tool creates a layout only. Prescription format and registration requirements vary by country and regulator, so check what applies to your practice.

Frequently asked questions

What should a prescription pad include?

A prescription pad normally carries the doctor's name and qualifications, the clinic or hospital name and address, contact details, and the registration or licence number issued by the medical council or regulator. It also leaves room for the patient's details, the date, the medicines prescribed and the doctor's signature.

Is the registration number required?

Requirements depend on your country and regulator, so check the rules that apply to you. The generator prints the number you enter as “Reg. No.” under the doctor's name. If you leave the field empty, that line is left out.

Can I customise the layout?

Yes. Choose from six templates (Classic, Banner, Corner, Sidebar, Wave and Minimal), any accent and text colour, three fonts, three text sizes and A4, A5 or US Letter paper. Add your clinic logo (PNG, JPG, WebP or GIF up to 2 MB) to the header or as a faint watermark, add a second doctor, and choose which patient details and page elements appear.

Are there layouts for specific specialties?

Yes. In step 4, choose a specialty layout: Paediatrics adds height, head circumference and dose-per-kg fields; Dentistry adds an FDI tooth chart; Physiotherapy adds an exercise programme table; Psychiatry adds sleep, mood, risk and mental state fields; Ophthalmology adds a spectacle prescription grid.

Can I type a prescription instead of printing a blank pad?

Yes. Choose “Write a prescription” at the top. Enter the patient's details, medicines (dose, frequency, duration and instructions), advice and follow-up date, and they print on your pad design. Patient details are not saved anywhere and disappear when you close the tab.

Can I make the pad in Hindi, Tamil, Arabic or another language?

Yes. Every field accepts any language or script, including Indian languages, Urdu, Arabic, Hebrew, Chinese, Japanese, Korean and Thai, and the text is shaped correctly in the PDF. Choose a pad language in step 1 to print the labels (such as “Patient name” and “Signature”) in 40 languages.

How do I print the prescription pad?

Use Print to open the PDF ready for printing, or Download PDF to save it. Print at 100% (actual size) on plain paper or your clinic's stationery. For bound pads, take the PDF to a local printer and ask for it to be padded.

Will my details be saved?

Your pad design and clinic details are remembered in this browser so you can come back and edit them. Nothing is uploaded to our servers, and the tool is free with no sign-up. Patient details typed in “Write a prescription” are never saved. Use “Clear all and start again” to remove everything.
